Combining clinical, radiologic, and pathological information is paramount to reach accurate diagnosis in interstitial lung diseases. Thorough comprehensive clinical history, physical examination , chest radiograph, high resolution CT scan and pulmonary function tests are required in evaluating these patients.
Exposure in environment / occupational exposure, smoking history, family history, medication history should be ascertained. After generating differential diagnosis, discriminating laboratory tests are performed. Other invasive tests like lung biopsy or bronchoscopy also done in evaluating these patients who require it. Interstitial Lung Diseases can be acute onset [eg: Hypersensitivity pneumonitis] , subacute onset [eg: Cryptogenic organizing pneumonia] and chronic onset [eg: Idiopathic pulmonary fibrosis].
ILD can be due to idiopathic causes or known secondary causes [eg: Connective tissue disease, medications induced]. Extrapulmonary symptoms and signs can give clues to the underlying cause of ILD. Patients with interstitial lung diseases present with progressive breathlessness, cough, chest pain and fever along with other symptoms depending on the underlying cause of interstitial lung diseases.
Specific diagnosis can be made in many patients from the results of a careful history and certain laboratory tests. Smoking related interstitial lung disease also requires special mention. Treatment is based on the underlying cause of ILD.
Interstitial lung diseases are associated with substantial morbidity and mortality. Early diagnosis and appropriate therapy can alleviate the suffering. There is significant scientific advances in the understanding of the various types of interstitial lung diseases in recent times.
